New Patient Information & History

When you arrive for your Tele-Eye Exam at The Eye Man, you'll begin by filling out a brief intake form on one of our devices. This step gathers important information about your medical history, ocular health, current medications, and allergies. Completing this ensures that we have a complete picture of your eye health before beginning your exam. Once your intake is complete, you’ll be assigned to a technician and an optometrist for your exam.
 

Pretesting

Next, your local technician will guide you through a series of pretests, explaining what each one measures and why it’s important for your eye health. Pretesting includes:

  • Autorefraction: Provides an estimate of your eyeglass prescription.

  • Ancillary Testing: Captures videos to assess pupil reaction and extraocular muscle movement.

  • Non-Contact Tonometry: Measures the pressure inside your eye, an important test for glaucoma detection.

Subjective Refraction

Following pretesting, you’ll be escorted to the exam chair where your technician will assist in setting up the phoropter -  the device used for your subjective refraction. During this part of the exam, the technician will guide you through a series of questions ("Which is clearer: one or two?") to fine-tune your eyeglass or contact lens prescription. Once complete, you will be placed into a virtual queue for the optometrist to begin the evaluation.
 

Exam Result Review & Analysis

Your optometrist will carefully review all data collected during pretesting and subjective refraction. They will assess your ocular health, verify the test results, and discuss any findings with you. After your exam, the optometrist will issue an updated prescription if necessary. If any condition is identified that requires specialized treatment, we will provide a referral to an appropriate specialist.
 

Prescription

Once the evaluation is complete, your technician will print your new eyeglass and/or contact lens prescription immediately.
